The TMF Personal Training Facility Fee is mandatory for all personal training clients. Facility fee payments may be paid in full for a term of 12 months or made monthly with a 12 month contract. Monthly facility fee payments require a contract of 12 months and may not be cancelled. A 90 day notice is required  to cancel only for 

extenuating circumstances, please contact stayfit@mad5fitness.com. You must come into the facility to cancel a membership due to extenuating circumstances.
